PERMISSION FORM FOR sophomores to attend AREA CAREER EXPLORATION (ace) day at fairmont high school on APRIL 9, 2025

Please note: Passenger limitations
Teen drivers are only allowed to have one passenger under the age of 20 for the first six months they have their provisional license, unless there is also a parent or guardian in the vehicle.
For the second six months they have their license, teen drivers are limited to no more than three passengers under the age of 20 without a parent or guardian also in the vehicle.
The limit does not apply for passengers under age 20 who are members of the driver’s immediate family.

My Sophomore student has permission to attend the Area Career Exploration (ACE) on April 9, 2025. I understand that they will be riding with MLHS staff and plan on leaving MLHS at 8:20 a.m. for 8:30 a.m. arrival at the Fairmont High School. All students are expected to return to MLHS after dismissal. (About 11:50 a.m.) My student will follow dress code parameters and wear business attire. (NO JEANS, SWEATS, CAPS, HOLES IN CLOTHES, CROCKS, SHORTS OR HOODIES--DRESS TO IMPRESS FOR SUCCESS!) Follow the Dress Code policy in the handbook. They will behave in a Christ-like manner and represent MLHS in the best way possible. Make any comments needed below. *
